Chronic Condition Management
At Salisbury Medical Centre, we have a Chronic Condition Management Team, which assists those patients who suffer from a long-term medical condition, helping them manage this into the future. Chronic disease can be a burden on not just individuals but families as well, which can be stress-inducing, leaving loved ones feeling confused and helpless. Chronic diseases are usually a condition that has been present for at least six months which can include:
- Kidney disease
- Diabetes
- Heart disease
- Asthma
If you have a chronic condition, we’ll create a personalised GP management plan to address your individual needs. To do this, you’ll first visit one of our chronic condition nurses, who will draw up a provisional plan. You’ll then visit the doctor, who will decide on the final healthcare plan and discuss this with you.
We also assist patients in accessing free services to help manage their chronic diseases. This includes GP Management Plans, medication reviews and regular reviews to ensure our patients are in good health. Our CCM clinic nurses are part of your healthcare team and will refer you to your required Allied Health provider(s).
