Your personal information
When you register as a patient of Salisbury Medical Centre, you provide consent for our GPs and practice staff to access your personal information.
We collect your name, date of birth, address, contact details, Medicare number, healthcare identifiers and health fund details. Medical information may include medical history and care required.
Your medical records
This practice takes steps to ensure that your medical records:
- are accurate, complete, well organised and legible
- are up to date
- contain enough information to allow any GP to care for you
- contain a summary of your care
- can be used to remind you, with your permission to return for follow up, check ups and reviews.
If you wish to remain anonymous while accessing healthcare services, please talk to practice staff
Providing your information to others
GPs respect your right to decide how your personal information is used or shared. Our practice will not share your personal health information with anyone else or another organisation unless:
- you have consented to sharing or
- there is a legal obligation to disclose information. Your GP will first discuss with you the information that she/he is legally obliged to disclose, or
- the information is necessary for you to obtain Medicare payments or other health rebates, or
- there is an overriding public health & safety interest
Security
All health records are kept secure using our medical software with password protection. Bookings made online use Hotdoc, which is a secure platform ensuring your details remain private.
How to access or update your health information
You have the right to request access to, and correction of, your personal information
To access your medical records please put a request in writing to our Practice Manager, Rebecca Bosomworth pm@salisburymedicalcentre.com.au
You will be required to attend a consultation with your GP.
Our practice will take reasonable steps to correct your personal information. From time to time, we will ask you to verify that your details, held by our practice, are correct. You may also request that we correct or update your information. Please put a request in writing to our Practice Manager, Rebecca Bosomworth pm@salisburymedicalcentre.com.au
How to lodge a privacy-related complaint
We take complaints and concerns regarding privacy seriously. You should address any privacy concerns you may have in writing. We will then attempt to resolve your concerns in line with our resolution procedure.
