Are you eligible for a GP Chronic Condition Management Plan?

Speak to your doctor to check if you are eligible for a GP Chronic Condition Management Plan. 
Recent changes to the criteria for GP Chronic Condition Management Plans, could mean that you are now eligible.  If you have diabetes, asthma, heart disease or another long term illness, speak to your doctor regarding eligibility. Under a GPCCMP, you can take a proactive approach to your health care by setting goals.  You will have access to up to five Medicare-subsidised allied health appointments per calendar year, such as for physiotherapy, dietetics, psychology, or exercise physiology. 
Reviewing your personalised  plan every three months with your GP,  will insure you are taking the best care of your health.
