What is a Chronic Disease?

Chronic diseases are usually a condition that has been present for at least six months which can include:


  • Kidney disease 
  • Diabetes 
  • Heart disease
  • Asthma
  • Mental Health Disorders
  • Cancer
  • Musculoskeletal conditions
  • Stroke


We have a Chronic Disease Management Team

To assists those patients who suffer from a long-term medical condition, we have a support team of health care professionals.

Effective management of chronic conditions can help improve your overall health and wellbeing. It can prevent or delay complications and slow the progression of your disease.


How can we help?

If you have a chronic condition, we’ll create a personalised GP management plan to address your individual needs. To do this, you’ll first visit one of our chronic disease nurses, who will draw up a provisional plan. You’ll then visit the doctor, who will decide on the final plan and discuss this with you.


We also assist patients in accessing free services to help manage their chronic diseases. This includes GP Management Plans, Team Care Arrangements, medication reviews and regular reviews to ensure our patients are in good health. Our CDM clinic nurses are part of your healthcare team and will refer you to your required Allied Health provider(s).
